If you are having difficulty adding your child(ren) on to the Healthy Holidays website, please contact the Healthy Holidays team (hhbooking@leeds.gov.uk) so that we can send you a different version of the form. You can also get in touch with your local holiday club or participating school and they can provide you with a registration form for your child.
